  The Registered Nurse (RN) in Home Health at Mederi Caretenders provides and oversees the delivery of nursing care to patients in their homes, as prescribed by physicians and in accordance with applicable laws, regulations, and agency policies. Additionally, the RN coordinates the total plan of care with other healthcare professionals involved in patient care and ensures continuity of care by planning and exchanging information with physicians, agency personnel, patients, family members, and community resources within a Point of Care setting.
  • Delivers clinical services within the scope of practice, as defined by state laws governing nursing practice, in accordance with the plan of care and in coordination with other healthcare team members.
  • Conducts initial and comprehensive nursing evaluation visits, determines the primary focus of care, develops the plan of care within State-specific guidelines, and submits accurate, complete, and timely documentation, per policy.
  • Regularly evaluates and re-evaluates (as warranted by changes in condition but at least every 60 days) the patient's nursing needs.
  • Performs patient comprehensive assessments at designated time points and develops the appropriate POC, in collaboration with physician orders.
  • Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.
  • Initiates, develops, implements, and makes necessary revisions to the plan of care in collaboration with the physician and other healthcare professionals involved in patient care.
    Required Qualifications
    • Current RN licensure in the state of Florida
    • Current CPR certification
    • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportationOur estimated annual salary for this position is around $85,000 to $110,000, depending on experience and qualifications.
